We understand life happens and that may mean your glasses get broken, scratched, or damaged in some way. In an effort to continue to help Hoosiers see better, look better, and feel better, we have made it easy for you to request a replacement of your eyewear by filling out the form below. How does the repair/replace form work?

Based on your submission, we’ll work with you to assess the damage to your glasses. We’ll then review any warranties you’ve pre-purchased, verify your prescription is still valid, and check in with your insurance to see if you have any available coverage. A representative from Team Tavel will call you directly to discuss your options.

What if my prescription is expired?

If you are in need of an exam, we’ll work with you to get you in for an appointment ASAP. You can schedule an appointment at your convenience by clicking here.

What if the prescription is from another provider (outside RX)?

If you have an expired prescription from another doctor, please contact our customer service team at 844-GO-TAVEL for additional input. You can schedule an appointment at your convenience by clicking here.

How much will this cost?

Our staff will reach out to you to confirm any out of pocket costs for your replacement eyewear. We will be sure to verify whether you have a warranty, insurance, or other benefits available.

Are you offering any deals on replacement pairs?

In the event that your original frame choice is no longer available, you may take advantage of our Clear Choice pricing, which includes a complete pair of glasses for $99.98. With this offer, you can choose any of our Vision Values® frames, and your lenses will be upgraded to the thinner, lighter, polycarbonate material, and we add on our standard Clarifier™ non-glare coating. Qualifying lens types include single vision, flat-top bifocals, or trifocals. If you choose to add additional upgrades to your glasses, additional fees will apply.  All new orders require custom measurements, which must be done in person by an Optician. If you would like to take advantage of this replacement offer, you must be able to visit your local Dr. Tavel during business hours.

I have the Tavel 20/20 Warranty – how does that work?

If you purchased a warranty, rather than requiring a complete repurchase of your prior glasses, we will replace them for a nominal fee: $20 for a replacement frame that originally retailed under $300, and an additional $20 for replacement lenses. So, your maximum out of pocket cost could be as low as $40. A customer service representative will be happy to confirm your anticipated out of pocket costs by phone. As a reminder, the 20/20 warranty can only be used a maximum of three times.

How do I get my frame and lenses to Dr. Tavel?

We ask that you visit your nearest Dr. Tavel location to finalize your replacement order. For those who are unable to visit a store in-person, we ask that you take photos of your broken materials and email them to [email protected].

  • If it’s a broken frame, but your lenses are still good. You do not need to send us your broken parts. However, you will need to visit a Dr. Tavel store to have your lenses reinserted into a new frame.
  • If your lenses are damaged, but your frame is fine, then you will need to visit a Dr. Tavel store to have your replacement lenses inserted into your existing frame.
  • If you are redeeming a 20/20 warranty, we require that you return your damaged eyewear – no matter the current condition – in order to use your warranty benefit.

How do I know if my insurance covers a replacement?

Our insurance experts will review your eligibility to determine whether you have replacement coverage. You will be notified either way when a customer service representative calls you.

How long will the replacement take?

Based on your individual prescription, as well as your prior frame selection, we will provide you with a customized timeline for your replacement. Most replacement requests take approximately 14 days to complete but

What if I want to add additional upgrades to my lenses?

Assuming your prescription is still valid, we are able to add additional material updates, coatings, Transitions, and any other lens treatments or options when you order replacement lenses. These upgrades will require additional out of pocket costs.

What if I’d prefer to order a completely new pair of glasses?

We would love to help you select a new, stylish pair of glasses. Stop into your local Dr. Tavel and our frame stylists will be happy to help you find your perfect pair of eyewear.
